Industry-funded study of the week: grains exonerated!

Perspective: Refined Grains and Health: Genuine Risk, or Guilt by Association? Glenn A Gaesser.  Adv Nutr. 2019 May 1;10(3):361-371.

Conclusion: This literature analysis illustrates a pitfall of attributing health risks to specific food groups based primarily on analysis of dietary patterns. With regard to refined grains, a large and consistent body of evidence from meta-analyses of prospective cohort studies suggests that the assumed health risks are largely a consequence of guilt by association with other foods within the Western dietary pattern, and not to refined grains per se.

Comment:  The author set out to counter a recommendation of the 2015 Dietary Guidelines Advisory Committee that to improve dietary quality, it’s better to replace most refined grains with whole grains.  Refining whole grains removes the great majority of their vitamins, minerals, and fiber (fortified flour replaces some of the nutrients, but not all).  Furthermore, refined grains are the main ingredients in many ultra-processed junk foods that promote overeating calories and raise risks for chronic disease.  Wheat per se may not be the problem, but what about the foods made from it?  I keep thinking: “grain-based desserts,” the number one contributor to calories in the American diet, according to the Dietary Guidelines.

Why do studies like this?  So the Texas Wheat Association can issue this headline: “New study exonerates refined grains.”

Food Navigator Special Edition: Pulses, seeds and grains

Food Navigator is an industry newsletter with articles about topics of interest to food companies.  This is one of its occasional collections of columns on a particular topic, in this case, beans, seeds, and grains.

Beans, chickpeas, peas and lentils are now appearing as added value ingredients in every part of the store, from chips and snacks to salads, soups, pastas, dips and baked goods. Non-GMO, gluten-free, high in protein, fiber and micronutrients, and low in fat, beans in particular are undergoing a PR renaissance among consumers, who have been eating them for years in tacos and burritos, but now see them as a more wholesome alternative to soy, rice, corn and potatoes in their snacks. So where will they go next, and who is driving innovation, both from a formulation, and branding perspective? We also take a look at ancient and ‘heirloom’ beans and grains.

General Mills wants the FDA to define “whole grain”

Ah the ironies of food marketing.  General Mills is asking the FDA to come up with a decent (translation: favorable to General Mills) definition of “whole grain.”

You might think that the meaning of “whole grain” is perfectly clear.  Wheat grains contain three parts, the bran, the germ, and the endosperm (the starch-and-protein part).

As I explained in a column for the San Francisco Chronicle, the FDA has not issued a rule defining whole grains.  Its has nonbinding guidance.  This says anything labeled “100 percent whole grain” must contain all three components of the original wheat seed, in proportion.

But what about products that are not 100% whole grain, which means most food products.  Here’s why General Mills cares about this issue:

Into this regulatory gap has charged the industry-sponsored Whole Grain Council, a trade association for marketers of whole grain foods.  The council issues two certifying stamps: 100 percent and Basic. One hundred percent fits the FDA guidance.

But the far more prevalent Basic stamp allows refined grains and not-necessarily-in-proportion additions of bran or germ.

General Mills wants the FDA to finalize its 2006 guidance).  This recommended:

  • At least 8 grams of whole grain per 30 gram serving for basic whole grain statements
  • At least 16 grams of whole grain per 30 gram serving for statements such as “100% whole grain”
  • All three components must be present in natural proportions

According to the account in Food Chemical News,  General Mills wants to head off the proposal from Center for Science in the Public Interest (CSPI); this would require posting the percentage of whole grains.

CSPI points out that many “whole grain” claims are misleading.  Without having to reveal the percentage of whole grain, companies can claim whole grains with only 8 grams of whole grains in a 30-gram serving.   This is 27% whole grain, meaning 73% not whole grain.

Yes, the FDA needs to act on this one, and the sooner the better.

San Francisco Chronicle column: whole wheat is not the whole story

My first-Sunday-of-the-month San Francisco Chronicle Q and A column, Food Matters, is out.  This month it’s about figuring out what “whole wheat” labels mean on food packages.

Q: I pay $4 for multigrain or whole wheat breads because I’ve heard white bread isn’t as healthy. But when I compare nutrition labels, $2 white breads look much the same. Are they?

A: My Talmudic answer: yes and no. You are asking about commercial sliced breads. Bread may be the staff of life, but you would never know it from reading the ingredient lists of most commercial products.

Commercial breads are indeed much the same, with only a few differences that matter.

To decide whether these have anything in them worth eating beyond their calories, you must inspect labels to make sure the first ingredient is whole grain, the total number of ingredients is small and devoid of unpronounceable chemicals, the fiber content is at least 2 grams per 1-ounce serving and the label says 100 percent whole wheat. Anything less is reconstituted white bread with occasional pieces of the original grain added back.

And then there is taste. Artisanal breads begin with just four ingredients – flour, water, salt and yeast – and turn them into loaves so crusty, chewy and fragrant that you cannot stop eating them. If they have some whole grain in them, even better.

But handmade breads take forever to make and quickly go stale. Commercial bakeries deal with these problems by rushing the bread-making process and compensate for the loss of flavor by adding stabilizers, dough softeners and preservatives, and covering up the chemical tastes with sweeteners. Breads with 30 or more ingredients are not unusual and violate my rule: Never buy processed foods with more than five ingredients.

To compare breads, you must read labels. Bread companies do not make this easy. Some list the serving size as one slice, some two, and their weights can vary by twofold. When you convert everything to ounces, the nutrient content of supermarket breads looks much alike.

An ounce provides 70 to 80 calories, a trivial difference. The grain is what counts.

Wheat grains have three components – the nutrient-rich bran and germ (“chaff”), and the endosperm, which is mostly starch and protein. One hundred percent whole wheat flour contains all three in the same proportion as in the original grains.

White flour contains about 80 percent of the original components. It is mostly endosperm.

Nutrients in the chaff are lost, so bakers are required to replace the five nutrients least likely to be available from other foods: niacin, riboflavin, thiamin, folic acid and iron. The others are not replaced.

Neither is fiber. White flour contains only trace amounts of fiber.

Because high-fiber diets promote healthy bowel function and appear to reduce risks of heart disease and bowel cancers, dietary advice is to eat at least three daily servings of whole grains – 3 ounces of 100 percent whole wheat bread, for example.

Food labeling rules do not make it easy to figure out fiber content. Some white breads list 1 gram of fiber, but watch out for serving size. It takes two slices to reach half a gram, which can be rounded up to 1.

Whole wheat bread with 2 grams of fiber per 1-ounce slice may have four times as much fiber as white breads. But watch out for breads listing 3 grams fiber; their slices may weigh nearly 2 ounces.

In response to dietary advice, commercial bakeries have introduced whole grain breads acceptable to white bread eaters. These grind the wheat bran super fine, add extra dough conditioners to keep the bread soft, and toss in some bran or cracked wheat to make the bread look like whole wheat. Check for fiber grams and the position of chaff ingredients on the list. The further down the list, the smaller their contribution.

And where is the Food and Drug Administration to help with whole grains? Alas, the FDA has not set rules for grain content. It permits manufacturers to make statements such as “100 percent whole grain” as long as the statement is true and does not imply that the food is an “excellent source.”

The FDA’s nonbinding guidance says anything labeled “100 percent whole grain” must contain all three components of the original wheat seed, in proportion.

This regulatory gap permitted creation of the industry-sponsored Whole Grain Council. The council issues a certifying stamp in two forms: 100 percent and Basic. One hundred percent means all grains are whole. But the more prevalent Basic stamp allows refined grains and disproportionate additions of bran or germ.

The Whole Grain Mess

Q.  Daniel from Ithaca writes: “I love whole grains as much as I love clear, honest labeling of foods. It is discouraging that even the WholeGrainsCouncil.org label on some foods is misleading. I just saw a loaf of Rubschlager Wheat Bread with the WGC symbol on it. It contains: Whole wheat, enriched wheat and 2 different sweeteners. I’m not sure how this is “whole”. What if foods with the Whole Grains Council or other proclamation that it is a “Whole Grain,” contained only Whole Grains with no “enriched flour” or added sugars?”

A.  The companies would probably go out of business; they wouldn’t have a clue how to make bread without all that stuff. The Whole Grains Council is a trade association/public relations agency that uses nutrition messages about the health benefits of whole grains to promote the products of its 160 member companies. One of its goals is “To promote whole grains through a positive message about their benefits, rather than by criticizing refined grains.”

The Council takes advantage of a gap in regulations; the FDA has not defined the meaning of “whole grains” on food labels but, instead, has produced Guidance for Industry. Examples:

Question: Does the term “whole grain” mean the same as “100 percent whole grain”? If a product is labeled as “whole wheat bagel” or “whole wheat pizza,” how much whole wheat should it contain? Answer: FDA has not defined any claims concerning the grain content of foods…We recommend that products labeled with “100 percent whole grain” not contain grain ingredients other than those the agency considers to be whole grains….We note that wheat flour should not be labeled as a whole grain flour because wheat flour is a synonym of flour…However, whole wheat flour (§ 137.200) should be considered a whole grain flour because it contains all the parts of the grain, i.e., the bran, endosperm, and germ….

Question: What types of label statements about whole grains are currently permitted to be made on food products? Answer: Manufacturers can make factual statements about whole grains on the label of their products, such as “10 grams of whole grains,” “½ ounce of whole grains,” (21 CFR 101.13(i)(3)) and “100% whole grain oatmeal” (as percentage labeling under 21 CFR 102.5(b)), provided that the statements are not false or misleading under section 403(a) of the Act and do not imply a particular level of the ingredient, i.e., “high” or “excellent source.”

Translation: The Whole Grains Council is doing a bit of an end run around the FDA. Is this a public service? You decide. Check out the Bread chapter in What to Eat and enjoy whole grains!

The food industry’s role in dietetic education

I frequently get sent copies of e-mailed messages sent to members of the Academy of Nutrition and Dietetics, the professional organization for people holding credentials as Registered Dietitian Nutritionists (RDN).

Disclosure: I do not hold this credential; long after I had a doctorate while I was doing a later master’s in public health, I fulfilled all of the graduate and clinical requirements but was still short one undergraduate course.

Holders of the RDN must complete 75 credits of continuing education courses every five years to maintain the credential.  Usually, dietitians must pay for courses to meet those credits.

But as my correspondents make clear, food complanies offer courses for credit—at no cost.

It is not at all difficult to fully meet credit requirements through industry-sponsored courses.

Comment: One can only imagine what those sessions will say.  Do not expect anything critical about the sponsor’s products.

Confused about diet? Oh, please.

I breathed a long sigh when I read Gina Kolata’s We’re So Confused: The Problems With Food and Exercise Studies in the New York Times on August 11.

Nearly everything you have been told about the food you eat and the exercise you do and their effects on your health should be met with a raised eyebrow… The problem is one of signal to noise. You can’t discern the signal — a lower risk of dementia, or a longer life, or less obesity, or less cancer — because the noise, the enormous uncertainty in the measurement of such things as how much you exercise or what exactly you eat, is overwhelming. The signal is often weak, meaning if there is an effect of lifestyle it is minuscule, nothing like the link between smoking and lung cancer, for example.

This is a pessimist’s view.  I, however, am an optimist.

Nutrition advice could not be easier to understand.  Eat plenty of vegetables and fruits; balance calories; don’t eat too much junk food.

David Katz must agree.  He asks: Diets, Doubts, and Doughnuts: Are We TRULY Clueless?

No, we are not, absolutely not, emphatically NOT clueless about the basic care and feeding of Homo sapiens…In the New York Times this past week, Gina Kolata made the case that almost all studies about diet, exercise, and health are suspect in one way or another, and that therefore we are confused about lifestyle practices for health, and justifiably so. The first point is valid; the second is utter nonsense…A global consensus of expert judgment concurs… Routine physical activity and a diet of mostly minimally processed vegetables, fruits, whole grains, beans, lentils, nuts, seeds and water when thirsty redounds consistently to the advantage of human health. It offers benefits to the planet as well.

Or, as Michael Pollan famously put it:

Eat food, not too much, mostly plants.

Articles like that one add to the confusion; they don’t help clear it up.
