Confused about diet? Oh, please.

I breathed a long sigh when I read Gina Kolata’s We’re So Confused: The Problems With Food and Exercise Studies in the New York Times on August 11.

Nearly everything you have been told about the food you eat and the exercise you do and their effects on your health should be met with a raised eyebrow… The problem is one of signal to noise. You can’t discern the signal — a lower risk of dementia, or a longer life, or less obesity, or less cancer — because the noise, the enormous uncertainty in the measurement of such things as how much you exercise or what exactly you eat, is overwhelming. The signal is often weak, meaning if there is an effect of lifestyle it is minuscule, nothing like the link between smoking and lung cancer, for example.

This is a pessimist’s view.  I, however, am an optimist.

Nutrition advice could not be easier to understand.  Eat plenty of vegetables and fruits; balance calories; don’t eat too much junk food.

David Katz must agree.  He asks: Diets, Doubts, and Doughnuts: Are We TRULY Clueless?

No, we are not, absolutely not, emphatically NOT clueless about the basic care and feeding of Homo sapiens…In the New York Times this past week, Gina Kolata made the case that almost all studies about diet, exercise, and health are suspect in one way or another, and that therefore we are confused about lifestyle practices for health, and justifiably so. The first point is valid; the second is utter nonsense…A global consensus of expert judgment concurs… Routine physical activity and a diet of mostly minimally processed vegetables, fruits, whole grains, beans, lentils, nuts, seeds and water when thirsty redounds consistently to the advantage of human health. It offers benefits to the planet as well.

Or, as Michael Pollan famously put it:

Eat food, not too much, mostly plants.

Articles like that one add to the confusion; they don’t help clear it up.

Israel’s solution to peanut allergies

While in Israel, I kept hearing that peanut allergies are virtually unknown in that country.  Nobody I met knows anyone with problems with peanuts—not Jews, Arabs, children, nor adults.

The explanation?

Bamba (and its Arabic equivalent).

These things are peanut puffs.

Because they melt in the mouth, they are often fed to babies as a first food.  Apparently, babies love them.

How do they taste?  Just as you might expect (peanut-flavored straw?  peanut-flavored Cheetos?).  They are sold everywhere as a snack and I met adults who love them too.

Do they really prevent peanut allergies?  Indeed, there might be something to this idea.

Introduction of peanut products into the diets of infants at high risk of developing peanut allergy was safe and led to an 81 percent reduction in the subsequent development of the allergy, a clinical trial has found…Researchers led by Gideon Lack, M.D., of King’s College London, designed a study called Learning Early About Peanut Allergy (LEAP), based on observations that Israeli children have lower rates of peanut allergy compared to Jewish children of similar ancestry residing in the United Kingdom. Unlike children in the UK, Israeli children begin consuming peanut-containing foods early in life.

Translation:  Bamba.

These researchers also found that allergic reactions did not return even if the children in that trial stopped eating Bamba for a year.

Their studies were funded mainly by NIH and the UK Department of Health, but the researchers also report that their clinical trials unit was supported by the National Peanut Board, Atlanta, and that the manufacturer of Bamba supplied the products.  The lead author, Gideon Lack, reports holding stock in DBV Technologies, the maker of Viaskin Peanut, a product that helps people with peanut allergies tolerate exposure to peanuts.

I’d like to see these studies repeated by fully independent researchers.

In the meantime, pediatric allergists are advising parents to let their babies eat peanut butter (but not peanuts because babies can choke on them).

These allergists—and the authors of the Bamba studies—participated in an American Academy of Pediatrics consensus statement:

There is now scientific evidence (Level 1 evidence from a randomized controlled trial) that health care providers should recommend introducing peanut-containing products into the diets of ‘‘high-risk’’ infants early on in life (between 4 and 11 months of age) in countries where peanut allergy is prevalent because delaying the introduction of peanut can be associated with an increased risk of peanut allergy.

If these studies are right, introducing babies to the widest possible variety of foods as soon as they can handle solid foods (usually at 4 to 5 months) may well help prevent allergies later on.

Addition

A reader just sent another paper from the same authors, this one a randomized trial of six allergenic foods given to breastfed babies at 3 or 6 months of age.  By one analysis, allergies developed 1 to 3 years later in 7.1% of the later-introduction group and 5.6% of the earlier-introduction group—a result that was not statistically significant.  By another, allergies were much lower in the early-introduction group (2.4% vs 7.3%), especially with respect to peanut and milk allergies.

Where are we on Golden Rice?

Golden Rice, genetically engineered to contain beta carotene, has long been the poster child for the benefits of GMOs—as witnessed by this Time Magazine cover of July 31, 2000.Golden Rice on Time cover

Beta-carotene is a precursor of vitamin A and the idea behind this rice was that it could—a conditional word expressing uncertainty—help prevent blindness due to vitamin A deficiency in areas of the world where this deficiency is rampant.

But vitamin A deficiency is a social problem.  Fruits and vegetables containing beta-carotene are widely available in such areas, but are not grown or consumed as a result of cultural or economic issues.  If they are consumed, people cannot absorb the beta-carotene cannot be absorbed because of poor diets, diarrheal diseases, or worms.

Here we are, 16 years after the Time cover, and Golden Rice is still not on the market.

I predicted its current problems in my book, Safe Food: The Politics of Food Safety, first published in 2003.  In Table 12 (page 158) I outlined the many basic research studies and research on production, consumer acceptance and use, and clinical effectiveness that would have to be done before Golden Rice could be shown to achieve its intended purpose.  Much of this research has now been done but plenty more still needs doing on getting it produced and into the mouths of people who most need its beta-carotene.

Proponents of the benefits of Golden Rice, however, complain that anti-GMO activists are responsible for keeping the rice off the market.

Not so, says an article in the Source, a publication of Washington University in St. Louis.  Based on what some of its researchers have just published in an article in Agriculture and Human Values, the Source quotes one of its authors:

The rice simply has not been successful in test plots of the rice breeding institutes in the Philippines, where the leading research is being done,” Stone said. “It has not even been submitted for approval to the regulatory agency, the Philippine Bureau of Plant Industry (BPI)…The simple fact is that after 24 years of research and breeding, Golden Rice is still years away from being ready for release.”

As I learned long ago, even the slightest skepticism about Golden Rice is perceived by uncritical proponents of GMOs as an attack on science and the entire food biotechnology enterprise.  If you publicly express doubt that Golden Rice can solve the vitamin A problem, you will be accused, as I have been, of responsibility for the illnesses and deaths of millions of children.

As the table in Safe Food makes clear, Golden Rice is a highly technical approach to solving a nutritional problem resulting from cultural and socioeconomic factors.

Such solutions do occasionally succeed.  The best examples I can think of are iodized salt to prevent goiter and water fluoridation to prevent tooth decay.  But both of these interventions address geographical mineral deficiencies, not deficiencies resulting from cultural prohibitions or poverty.

Is Golden Rice worth a try?  Sure it is.  But not when it is used to demonstrate that GMO foods are good for the public as well as the owners of seed and pesticide companies.

Weekend reading: Science & Nutrition Research

I’ve just started getting a new Science and Research newsletter from William Reed: Informing Business Growth.

Some of the listings focus on dietary patterns or emerging microbiome research:

But most of them focus on single nutrients, ingredients, or foods—suggesting that they are about marketing those ingredients, not necessary health (people don’t eat single foods exclusively; we eat meals and mixtures).

These are a lot of fun.  They raise the possibility of magic bullets (if only).

But watch out for the weasel words “may,” “could,” “might”.  These mean “may or may not,” “could or could not,” “might or might not.”

Wouldn’t you rather eat chocolate than beets?  That’s what such studies are about.  And I wonder how many of them are funded by the makers of the products.  Want to take a look?

Does glyphosate (Roundup) cause cancer?

Glyphosate is an herbicide made by Monsanto to be used on crops genetically modified by Monsanto to resist it.  Growers can spray glyphosate on their crops.  When it works well, weeds die and the crops flourish.

It is widely used in production of genetically modified crops (HT—herbicide tolerant—in the figure).

Monsanto says it has many benefits and is risk free.

But in March 2015, The World Health Organization’s International Agency for Research on Cancer (IARC) said that glyphosate/Roundup is “probably carcinogenic to humans” (see my post on this).

Now a joint WHO/FAO meeting on pesticide residues concludes

glyphosate is unlikely to pose a carcinogenic risk to humans from exposure through the diet.

How to reconcile these divergent conclusions?

The Guardian says one possible explanation lies with who participated in the WHO/FAO meeting.  It notes that the meeting’s chair is vice-president of the International Life Science Institute (ILSI) Europe.  ILSI positions itself as an independent research group, but SourceWatch considers it a lobbying group and some critics view it as a front group for the food industry.  Says The Guardian:

In 2012, the ILSI group took a $500,000 (£344,234) donation from Monsanto and a $528,500 donation from the industry group Croplife International, which represents Monsanto, Dow, Syngenta and others, according to documents obtained by the US right to know campaign…the news sparked furious condemnation from green MEPs and NGOs, intensified by the report’s release two days before an EU relicensing vote on glyphosate, which will be worth billions of dollars to industry.

Even if questions about the carcinogenicity of glyphosate/Roundup are in dispute, one issue is not; weeds are increasingly developing resistance to the herbicide.  Farmers are forced to use other, perhaps more toxic, herbicides to get rid of resistant weeds.

The molecular basis of glyphosate/Roundup resistance is well understood, and more and more weeds are developing resistance.

If enough of them do, farmers will stop using glyphosate/Roundup and the carcinogenicity issue will become moot..

Let’s hope IARC and independent WHO/FAO committees are taking a close look at the potential carcinogenicity of all those replacement herbicides.

Weekend reading: Miraculous Abundance [Permaculture]

Perrine and Charles Hervé-Gruyer.  Miraculous Abundance: One quarter acre, two French farmers, and enough food to feed the world.  Foreword by Eliot Coleman.  Chelsea Green, 2016.

This book, more about philosophy than a how-to, describes how two inexperienced beginners succeeded in creating a gorgeous, productive, self-sustaining farm on 1000 square meters of land in Normandy—La Ferme du Bec Hellouin.

They did this by using the techniques of permaculture.  This they define as “a box of smart tools that allows the creation of a lifestyle that respects the earth and its inhabitants—a practical method inspired by nature.”  Later, they explain that it is based on an ethic: “Take care of the earth. Take care of the people.  Equitably share resources.”  As I said, philosophy, not how-to.

You have to read the book to figure out what all this means in practice.  It seems to come down to what I thought of as French Intensive methods.  These use raised beds, rich soil, composting, and thoughtful planting of coordinated crops that support each other’s growth and nutritional needs.  Vandana Shiva’s Navdanya—nine seeds—approach works the same way.   The authors drew on the work of John Jeavons, Eliot Coleman, and many other small-scale sustainable farmers from all over the world to develop their version of these methods.

If the color photographs are any indication, the results are magnificent.   The place is so highly productive that it easily supports the two of them.  The mandala garden alone made we want to get on the next plane just to see how it works in controlling weeds.

The moral: you could do this at home.

Healthy? Natural? It’s up to the FDA.

The terms “healthy” and “natural” help to sell food products.  They are about marketing, not health.

This makes life difficult for the FDA, which has the unenviable job of defining what the terms mean on food labels.

In a victory for the maker of KIND bars, the FDA has just said that the bars can be advertised as healthy—and that the agency will be revisiting its long-standing definition of the term.  This is what that definition says now:

You may use the term “healthy” or related terms as an implied nutrient content claim on the label or in labeling of a food that is useful in creating a diet that is consistent with dietary recommendations if the food meets the conditions for total fat, saturated fat, cholesterol, and other nutrients…In addition, the food must comply with definitions and declaration requirements for any specific NCCs [Nutrient Content Claims].

The chronology :

As reported by Food-Navigator-USA (in a remarkably thorough account of these events),

Dr. Susan Mayne, Director of the Center for Food Safety and Applied Nutrition at the FDA, said: We do not object to the specific statement that you would like to place on your bar wrappers, on the condition that there will be no other nutrition-related statement, such as express or implied nutrient content claims, on the same panel of the label…We agree with you that our regulations concerning nutrient content claims are due for a reevaluation in light of evolving nutrition research.”

What this sounds like is that FDA will be soliciting comments on the meaning of “healthy.”   It also sounds like the FDA agrees that fat is not an appropriate criterion.  But will the FDA set a limit on sugars?  KIND bars are sweetened.

This looks like the FDA will request comments as the start of its interminable rulemaking process.

Natural

The FDA is further along in that process for “Natural.  The comment period closed and Politico Pro Morning Agriculture reports that more than 5000 came in.  These have not yet been posted, but Morning Ag has some.  It says opinions vary.  Widely.

  • FDA should prohibit using the term.
  • Acceptable post-harvest processing and production methods [including GMOs]
  • No chemicals, no additives, and no kitchen chemistry
  • Some forms of processing can be used – and indeed may be necessary.
  • ‘Natural’ means that this product contains no artificial or synthetic ingredients.

I’ve commented many times in the past on the ongoing debates about “natural.”

I repeat: When it comes to food labels, “healthy” and “natural” are marketing terms.  Their purpose is to sell food products.

Caveat emptor.

Weekend reading: Krishnendu Ray’s The Ethnic Restaurateur (Bloomsbury, 2016)

Full disclosure.  I recruited Krishnendu Ray to NYU (for good reason as you can see from this interview in the Washington Post) and he is now my department chair.

With that said, I greatly admire what he’s done in this book, which is to cast a sociological eye on immigrants to the United States who get their start by using what they know of their own food tastes and traditions to open and run restaurants of the “ethnic” variety in today’s terminology.

Ray argues here (and elsewhere) that the contributions of immigrants to modern food culture are largely ignored by academics and critics who view

discussions of taste as marginal to the real lives of marginal peoples. In this conception, poor, hard-working people can teach us about poverty and suffering, hierarchy and symbolic violence, but never about taste…As a consequence, taste loses its contested and dynamic character, and…even its fundamentally sociological nature. As labor and immigrant historians have shown us repeatedly, good food matters to poor people, perhaps even more than it does to the rich and the powerful.

In his book, Ray draws on his readings, experience teaching at the Culinary Institute of America, and on interviews with cooks from China, India, Italy, and elsewhere to examine their motivations, experiences, and attitudes about the food they prepare and serve.  He says

Nothing devalues a cuisine more than proximity to subordinate others.  That explains not only the rise, fall, and rise again of Italian cuisine in America, but also the difficulty of Chinese, Mexican, and Soul food to break away, in dominant American eyes, from the contamination effect of low-class association.  Poor, mobile people are rarely accorded cultural capital.  The circulation of taste through the social architecture of class and race allows for the creation of a subcultural niche, say for the best taco, genuine dim-sum, or most authentic fried chicken, yet rarely assures a position among elite food cultures. [p. 97]

After reading this book, I find myself paying much more attention to the ethnic restaurants in my neighborhood, and thinking about who owns them, who works in them, and why and how they arrived at their menus.  This book will change the way you think about them too.
